Nevertheless, low work capacity, defined as maximum Vo2 of less than 80 percent predicted, was found in 15 of 16 patients. Maximum Vo2 was less than 50 percent predicted in 7 of 16 patients. In 107 patients with idiopathic pulmonary fibrosis and 42 patients with sarcoidosis, Keogh et al compared resting pulmonary function and arterial blood gas values with the frequency of abnormal Po2 response during exercise. In this study of 53 patients with idiopathic pulmonary fibrosis who had resting Pa02 less than 80 mm Hg, all had a decreased Pa02 during exercise, an abnormal response. Matthews and Hooper defined abnormal exercise response in patients with sarcoidosis, including abnormal respiratory frequency or arterial 02 desaturation. Resting pulmonary function tests had 92 percent sensitivity for predicting abnormal exercise response but only 47 percent specificity. This translated into 52 percent positive predictive value of resting studies, although the negative predictive value was 90 percent in this article.
