Subclinical Cardiac Dysfunction in Sarcoidosis (15)These results suggest that the reasons for the abnormal cardiac response during exercise are not simple and more than one explanation should be sought. Regardless of the cause, however, it seems likely that impaired cardiac function is a limiting factor to exercise in patients with sarcoidosis.
In previous studies, rapid exercise HR responses in sarcoidosis have been attributed to low right ventricular stroke volume resulting from exercise-induced pulmonary hypertension. Pulmonary hypertension could result from granulomatous and fibrotic obliteration of the pulmonary capillary bed and/or arterial oxygen desaturation. We do not believe this explanation applies to the majority of our patients since they had only mild, if any, pulmonary function impairment. Only one desaturated during exercise. Resting LVEF and HR responses during exercise were not related to the lung function or roentgenographic abnormalities. Several factors should be considered since they may complicate the interpretation of our results. First is the potential effect of deconditioning. Poor fitness alone is associated with reduced maximum oxygen consumption and rapid HR responses during exercise.
